COVID-19 Mandatory Vaccination Policy Acknowledgement
I understand that I am required to receive a COVID-19 vaccination as a condition of
employment, unless a medical or religious exemption is approved.
I recognize that this policy is divided into two phases. To be in compliance with Phase
1, I need to have started the vaccination process, previously completed the vaccination
series, or filed a request for medical or religious exemption on or before December 4th,
2021. If I do not comply, I understand that I will be placed on an unpaid leave of
absence until compliance is achieved. I may request to use PTO for this leave, if
available.
To be in compliance with Phase 2, I need to have completed either the single dose or
two dose series of an approved COVID-19 vaccine or have received an approved
request for a religious or medical exemption from Hiawatha HomeCare on or before
January 4, 2022. If I do not comply, I understand that I will be placed on an unpaid
leave of absence until compliance is achieved. I may request to use PTO for this leave,
if available.
Employees new to Hiawatha HomeCare will be required to be in compliance with this
policy prior to providing direct client care.
